The Kinship Services Support Worker has the responsibility and duty to ensure the needs of the child(ren) are met in kinship living arrangements, when the child is not considered to be in care of the Agency. The Kinship Service Support Worker is responsible to maintain a consistent focus on the needs of the child and advocates with the Intake Unit and/or Family Service Unit to achieve permanency for children living with members of the child’s extended family or community.

The Family Wellness Worker will provide guidance and support to families in order to promote safety, success and well-being for families. The Family Wellness Worker is responsible to work with families that require additional support and guidance in order to maintain family unity or reunification in a manner that respects and incorporates the concept of family centered, culturally appropriate, and community-based care. The Family Wellness Worker will complete kinship searches to facilitate permanency with the intended outcome to prevent admission to care and to expedite the return of the children recently placed in care. The Family Wellness Worker is responsible for providing the protection of children by performing duties of Intake, Protection, and Emergency after Hours, and Kinship as required, in accordance with Provisions of Dilico Anishinabek Family Care’s Mikinaak Service Model, the Child, Youth and Family Services Act, Ministry Regulations and Standards, and Dilico Anishinabek Family Care policies and procedures.

Under the direction of the Adult Transitional Housing Services Manager, the Cultural Wellness Mentor is responsible for ensuring the cultural needs of youth/adult clients are met who are on the Home for Good case management and waitlist. This includes managing the Home for Good client waitlist; opening new referrals; regular appropriate documentation on all client files; maintaining regular contact and meeting with clients; assisting with client appointments or needs; attend bi-weekly case review meetings, provide daily traditional healing, guidance, teachings, sacred circles and traditional ceremonies for youth participants within Adult Transitional Housing services. The Cultural Wellness Mentor is responsible to work collaboratively with the team to ensure program delivery is consistent with the philosophy, approach, goals and learning environment established by Dilico Anishinabek Family Care which will ensure the cultural, social, emotional and physical wellbeing of children/youth and their families.

The Alternative Care Worker is responsible for providing the protection and care of children by performing duties of Alternative Care and Adoption as may be required, in accordance with provisions of the Child and Family Services Act, Ministry Regulations and Standards, Abuse Protocols, and Dilico Anishinabek Family Care policies and procedures.

The Youth Intensive Treatment Counsellor will ensure daily needs of youth in care of the program are met by actively providing daily therapeutic opportunities in the twenty-four hour home setting. The Youth Intensive Treatment Counsellor is responsible to work collaboratively with the clinical team to ensure program delivery is consistent with the philosophy, approach, goals and learning environment established by Dilico Anishinabek Family Care, which will ensure the cultural, social, emotional, mental, and physical wellbeing of children/youth and their families.

Position Overview:

The Community and Personal Support Worker is a multi-functional para-professional who is responsible for the delivery of in home support and personal care to ill, frail, and persons with disabilities of all ages so that they may remain independent in their own homes and communities for as long as possible. With supervision provided by Dilico’s Community and Personal Support Services Managers, the Community and Personal Support Worker collaborates with the Home and Community Care Registered Nurse and other Health staff, to ensure the provision of culturally appropriate support and assistance to eligible clients including provision of support with activities of daily living, personal care, caregiver relief, palliative care, speech and language pathology support, adult life enrichment program services and basic foot care. If not located at identified office, can travel from another office. Dilico provides a vehicle rental and accommodations with meal reimbursement.
